Please call your hotel to make a reservation.
At that time, please be sure to tell us that you will use the health insurance association's subsidy system.
After the reservation is confirmed, the hotel will send a "reservation confirmation" by fax or email.

Please print out the “Contracted Resort Facility (Accommodation) Application Form” from the Health Insurance Society website (Recreation/Sports).
After filling in the necessary information, please attach the “reservation confirmation form” from STEP 1 and apply to the Health Insurance Society.
*Please apply well in advance of the deadline.

Once you have received approval from the Health Insurance Society, please send the "Approved Use Application Form" to the hotel Reservations desk by fax or email.

On the day of your stay, please bring the "Approved Use Application Form (original)" and hand it to the front desk staff at check-in.
Please pay the amount after deducting the subsidy amount from the usage fee.

Please contact the hotel where you made the reservation by phone.

<In case of cancellation>
After contacting the hotel by phone, please return the "Approved Use Application Form (original)" to the Health Insurance Society.

<In case of change>
Please submit the "reservation confirmation" reissued by the hotel when the change is accepted and the "approved use application form (original)" to the health insurance association, and reapply.

If the health insurance association approves your reapplication, please fax the “corrected and approved application form” to the Reservations desk.

On the day of your stay, please bring the "corrected and approved usage application form (original)" and hand it to the front desk staff at check-in.
Please pay the amount after deducting the subsidy amount from the usage fee.
